چکیده
BackgroundStudies on the value of a staging laparoscopy in detecting metastases gastric cancer patients show great variation. This study investigates avoidable surgery rate with and without scheduled for curative intent.MethodsThis population-based cohort included all an intentional resection potentially curable adenocarcinoma, between 2011 2016, registered Dutch Upper GI Cancer audit. Patients were compared. The primary outcome was (detection and/or locoregional non-resectable tumor during gastrectomy). Secondary outcomes negative predictive value, postoperative morbidity pathology parameters.Results2849 who underwent gastrectomy included. 414 2849 (14.5%) before initiation treatment. 16.2% group, compared to 8.5% non-staging group (P < 0.001), resulting 83.8%. remained significantly different after correction possible confounders. main reason not executing presence distant metastasis both groups. cT cN stage higher laparoscopy.ConclusionsThe had cTN pTN stage, implicating selection more advanced disease laparoscopy. Despite laparoscopy, found, suggesting low sensitivity or non-resectability this patient group.

Impact of comorbidities on postoperative complications in patients undergoing laparoscopy-assisted gastrectomy for gastric cancer
BACKGROUND Comorbidity is a predictor of postoperative complications (PCs) in gastrectomy. However, it remains unclear which comorbidities are predictors of PCs in patients who undergo laparoscopy-assisted gastrectomy (LAG). Clinically, insufficient lymphadenectomy (LND) is sometimes performed in high-risk patients, although the impact on PCs and outcomes remains unclear. METHODS We retrospec...
متن کاملStaging laparoscopy for gastric cancer.
The growing role of multimodal treatment plans for advanced gastric cancer has contributed to the development of more accurate preoperative staging strategies. The high diagnostic efficacy of video-laparoscopy as regards the M factor has been reported by many; preoperative laparoscopy therefore permits to avoid unhelpful surgical exploration in case of peritoneal dissemination of tumor or liver...
متن کاملThe CHOLEGAS study: multicentric randomized, blinded, controlled trial of gastrectomy plus prophylactic cholecystectomy versus gastrectomy only, in adults submitted to Gastric cancer surgery with curative intent
BACKGROUND The incidence of gallstones and gallbladder sludge is known to be higher in patients after gastrectomy than in general population. This higher incidence is probably related to surgical dissection of the vagus nerve branches and the anatomical gastrointestinal reconstruction. Therefore, some surgeons perform routine concomitant cholecystectomy during standard surgery for gastric malig...
متن کاملSurvival benefits of gastrectomy in gastric cancer patients with stage IV: a population-based study
Objective The aim of the current study is to investigate the role of gastrectomy for survival among metastatic gastric cancer patients. Results We finally identified 12,986 eligible patients with stage IV GC between 2004 and 2012, including 1,981 (15.3%) patients with gastrectomy and 11,005 (84.7%) without surgery. The median overall survival time for patients with and without surgery were 9....
متن کاملStaging laparoscopy in the management of gastric cancer: a population-based analysis.
BACKGROUND Staging laparoscopy can detect radiographically occult peritoneal metastases and prevent futile laparotomy in patients with gastric adenocarcinoma. We sought to assess the use of staging laparoscopy for gastric adenocarcinoma in a cohort of older patients and to compare outcomes after laparoscopy alone with nontherapeutic laparotomy. STUDY DESIGN Using Surveillance, Epidemiology an...
